To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. This member of the ACT multidisciplinary team has the responsibility to carry out treatment, rehabilitation and support functions, education, and provide crisis intervention to participants and support and consultation to families. Provide service coordination (case management) for an assigned group of participants including coordinating and monitoring the activities of the individual treatment team.

Responsibilities

SPECIFIC DUTIES:
1. Participate in the client-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living a (e.g., self-care, living situation, nutrition, money- management); and family structure and relationships.
2. Conduct comprehensive substance use disorder assessments as indicated.
3. Consult with community agencies and families to maintain coordination in the treatment process.
4. Participate in daily staff organizational meetings and treatment planning review meetings.
5. Participate in providing treatments and dual diagnosis services.
6. Assist in the provision of ongoing assessment of clients’ mental illness and substance use symptoms and clients’ responses to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in mental status or behavior that put clients at risk (e.g., suicidal).

Qualifications

*M.A. degree in Social Work, Psychology, Human Services/Social Sciences, Healthcare or related field.
*Internships and/or other supervised practical experience in a clinical or rehabilitation setting with persons with mental illness, substance use disorder, and/or co-occurring disorders.
*Certified Alcohol Drug Counselor preferred or License eligible
* Ability to use an electronic medical record
* Must be available or scheduled emergency on-call service and will be required to carry a mobile phone during regularly scheduled working hours and scheduled on-call service. As essential personnel may be required to report to work during emergencies including inclement weather
