This position is responsible for providing case management services to drug and/or alcohol eligible clients so that a more satisfactory social, economic, emotional or physical living condition can be achieved and a drug/alcohol free lifestyle can be maintained. This may include providing case management services to specialized populations, including those involved in the Chester County Treatment Court.

Responsibilities

• Conduct and/or review client intakes and assessments to approve and/or facilitate appropriate treatment placement.
• Directly provide and/or ensure development of service plan which addresses client support service needs. Responsible for ensuring that plan is routinely updated in compliance with Department policy.
• Directly provide and/or ensure referral and linkage to support services. Advocate for the client, provide support and encouragement while in case management.
• Monitor treatment progress, routinely assess appropriateness of treatment and service plans, conduct continuing stay, transfer and discharge reviews per Department policy.
• Coordinate addiction treatment and ancillary services.
• Assist clients with acquisition and coordination of Medicaid and other benefits.
• Data collection and reporting in compliance with department, state and/or federal requirements.
• Complete mandated documentation and maintain client file in compliance with Department policy.
• Provide outreach as assigned and follow-up as assigned to ensure client engagement.
• Participate in inter-disciplinary/inter-department teams as assigned which may include attendance at treatment court meetings and other various initiatives throughout Human Services.
• Work with all relevant parties including, but not limited to, court related departments, Chester County Prison, treatment providers and other service systems to ensure appropriate and coordinated services for clients.
• Communicate system issues/concerns to appropriate departmental staff for review and resolution, as needed.
• Compile, complete and provide reports on various data as required.
• Perform all other duties, tasks and special projects, as assigned.

Qualifications

• One year as a Drug and Alcohol Case Management Specialist Trainee; or
• One year of direct service experience performing counseling or case management work in an alcohol or other drug treatment setting; and a bachelor’s degree from an accredited college or university in chemical dependency, sociology, social welfare, psychology, nursing or a related field; or
• A bachelor’s degree from an accredited college or university that includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences and one year of direct service experience performing counseling or case management work in an alcohol or other drug treatment setting: or
• Any equivalent combination of experience and training.
