Center for Family Services is looking for individuals to join our Community Health Work (CHW) team. Interested applicants must be ready to join a fun, family oriented team that works diligently to assist, support and strengthen our local community. The incumbent of this position will be a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community. Our goal is to facilitate access to services and improve the quality of life and help build sustainability for those we serve.

Given the high prevalence of traumatic stress among the families we serve, it is likely many individuals are hyper-vigilant about perceived threats. This often places barriers for many to make progress in achieving their identified goals. It is the Center's goal to work with individuals and families to not only provide a safe environment but, also assist participants in accessing Social Determinant of Health (SDoH) resources. We aim to eliminate triggers through education, address trauma and actively create a safe physical and psychological experience while assisting individuals navigating the often difficult web of social services.

Responsibilities

RESPONSIBILITIES:· CHW's work within Crawford County, PA in close cooperation with local social service agencies, shelters, Crawford County Human Services and many other community resources to meet the needs of our participants. The incumbent will meet with participants to assess needs, including; helping to set Social Determinant of Health domains; determine CHW support; coordinate/monitor plan; and complete tasks as necessary to complete Social Determinants of Health domains within timely deadlines. Link / refer Participants with services to relieve underlying Social Determinant of Health Needs.

CHW must be able to Work in a team approach in partnership with participants, providers and other team members to promote timely access to care, continuity of care, and enhancement of participants experience and their assessment of overall health.

This is not a remote position. CHW will be provided office space at the Meadville, PA agency site. CHW are expected to meet with participants within the home, community and office.

Qualifications

Current Driver License Required, Previous Case Management experience preferred
